Has anyone one here heard of it happening to someone they know? How did it happen? Was it because of online banking?

The latest research shows most identity theft occurs due to carelessness by government agencies and financial institutions. While the media makes a big fuss, the figures are statistically insignificant.
Insurance agents have pounced on it as a new product they can sell.

No institution can guarantee 100% that they won't be hacked. It isn't necessarily carelessness on the institution's part but rather a criminal hacked into their system. The insitution is also a victim of a crime. My DH is a security engineer and deals with this stuff on a daily basis.

Never disclose to anyone your social security number, mother maiden name, birth date, use strong password for your email account and online bank sites. Never open any spam mail, protect your system from possible fraud attempt by using anti spyware and update/scan your PC regularly.

Thanks Dave, in addition also it is necessary to update regularly security software in your computer and use renowned anti spyware, don't store those personal information in your computer, keep your PC password protected as well.
